Always alert on honeypot and use CSPRNG

Honeypot handling now records and sends modlog alerts even when banning fails, including `banFailed`/`banError` metadata and a new embed variant that clearly flags manual moderator action. Captcha generation was also hardened by switching answer/operation randomness from `Math.random()` to `crypto.randomInt`, with tests updated to match the new behavior.
