Harden selector conflict recording per fresh-agent audit (bedrock-v2c)

Add defensive handling for a selector resolving to a key with a nil
value (records the scanned span and caches the miss, restoring parity
with the old execute_get_query keyed-nil path) and pin the
anchor==resolved single-key span.
