wire Coveralls: per-file coverage through test.seq

mix test.seq --cover exports a distinctly named .coverdata per test
file (full-path-derived — bare basenames collide across test/ and
test/nif/), preserving the one-OS-process-per-file isolation the
bundled-SQLite VFS demands. CI's new coverage job merges them via
`mix coveralls.github --import-cover cover <one file>` (a single
file per BEAM stays within the isolation rule) and pushes with
GITHUB_TOKEN. Coverage badge added; coveralls.github registered in
preferred_envs (was silently running in dev env otherwise).
