ci(nuget): sign packages with SignPath before publish; docs: add README badges (#13)

Insert a SignPath signing request between `dotnet pack` and the NuGet push
in the release pipeline so published .nupkg files are code-signed, and add
CI/coverage/NuGet/license badges to the README.
