Add opt-in OpenTelemetry auto-instrumentation

- setup.cfg: `opentelemetry` extras group (SDK 1.36.0 / contrib 0.57b0):
  OTLP http exporter + pyramid, sqlalchemy, requests, botocore, urllib3
  instrumentations (opensearch-py traffic surfaces via urllib3)
- src/igvfd/otel.py: per-worker SDK init, gated by OTEL_ENABLED
- gunicorn development.conf.py: post_fork hook (SDK must init after the
  fork; the opentelemetry-instrument wrapper breaks under pre-fork) and
  env-configurable worker count (GUNICORN_WORKERS, default 9 as before)
- docker-compose.otel.yml: opt-in override wiring OTLP env toward the
  workspace ClickStack collector; base compose file untouched
- Dockerfiles: install the new extras; optional EXTRA_CA_CERT_B64 build
  arg to trust a TLS-intercepting proxy (no-op when unset)
