refactor: share Livebook validation between MCP and test task

- Extract headless Livebook parser/evaluator into Choreo.Livebook
  and Kino mocks into Choreo.Livebook.KinoMock
- Choreo.MCP.choreo_verify_design now evaluates blocks, not just parses
- Code-fence-aware section parsing and exact header replacement
- Fix Mix.install stripping, empty Kino.Input.select, and runtime
  defimpl handling (consolidate_protocols: false)
- Skip integration livebooks needing external deps/project features
- Clean up xref_graph.dot artifacts and ignore them
- Add MCP section to README with client config and example prompts
