fix: invalidate attributes on disconnect; report the disconnect first

GATT handles are only valid within a connection, so a disconnect now bumps the
discovery generation (like discoverServices/services-reset) and every held
attribute becomes stale. requireValid checks the connection first, so using a
stale attribute after a disconnect throws deviceDisconnected rather than the
(misleading) 'superseded by re-discovery' message.
