Flush tool-filter buffer after handler returns (#5809)

* Flush tool-filter buffer after handler returns

When both the tool-filter and authz middleware are active, tools/list
responses came back with committed headers but a 0-byte body. Authz
writes the final body into the tool-filter's buffering writer during
its own post-ServeHTTP flush, but nothing then drains that buffer, so
it is stranded. Mirror authz's own pattern by explicitly flushing the
tool-filter writer after next.ServeHTTP returns.

Fixes #5797

* Reset buffer in no-content-type Flush branch

Prevents a repeated Flush() call on an unlabeled response from
rewriting already-flushed bytes, mirroring the reset already done
in the content-type branch below it.
