Ignore default integration repository when Agent Control manages integrations

Add an `agent_control_managed` config option (env: NRIA_AGENT_CONTROL_MANAGED)
that, when enabled, stops the agent from scanning the default
custom-integrations repository under agent_dir (e.g.
/var/db/newrelic-infra/custom-integrations). This avoids loading stale or
conflicting integrations from the old path when Agent Control manages
integrations via NRIA_CUSTOM_PLUGIN_INSTALLATION_DIR / NRIA_SAFE_BIN_DIR.

The option defaults to false, so existing installs are unaffected.

The integration management mode ("standalone" or "agent-control-managed") is
logged at INFO on agent startup.
