test: raise server coverage (audit, grpc, api-rest, core) + coverage gate & recipes (#184)

* docs: add test-coverage improvement plan (server + 7 SDKs)

Executable plan to raise coverage: axiam server 81% -> >=90%, all SDKs
to >=93% (Python toward 96%), plus per-repo CI coverage-threshold gates.
Phases are per-repo and independently executable by later agent sessions.

axiam-audit/src/middleware.rs 61%->92% lines, notification.rs 85%->89%:
- JWT extraction via Bearer header and axiam_access cookie fallback
- cached-identity reuse, non-bearer scheme, empty creds, invalid token
- worker survives repository errors and exits when senders drop
- entry-drop path when the audit channel is closed
- notification dispatch: non-matching-event skip and publish-error swallow

axiam-api-grpc/src/server.rs 0%->74% (plaintext boot path), services/user.rs
73%->100%, middleware/rate_limit.rs 79%->83%:
- start_grpc_server plaintext boot via timeout race (new grpc_server_test.rs)
- build_grpc_governor_layer construction + zero-rate panic + shared-layer clone
- UserService: all status_to_string arms, internal-error mapping, missing
  claims on validate_credentials, pepper path, bad-hash + failed-login errors
