ci(coverage): add merged line-coverage floor gate (C# SDK) (#11)

Both test projects instrument the shared Axiam.Sdk assembly, so a line covered
by EITHER run counts (union) — the same fold Coveralls applies. Current merged
line coverage is 93.7% (obj/ generated gRPC stubs excluded via ExcludeByFile);
the gate fails below a 92% floor. Remaining gap is AxiamAmqpConsumer.StartAsync,
which needs a live broker (no RabbitMQ in CI).
