test(ts): cover authz error mapping + add vitest coverage thresholds (#12)

- authzErrors.test.ts: non-403 status, batchCheck catch, and no-response
  NetworkError branches (authz.ts 86%->100% lines)
- vitest.config.ts: coverage thresholds gate (lines 93 / statements 92 /
  functions 95 / branches 84); overall lines now 95.6%
