fix(plugin): wording-agnostic stale subagent-stop detection (OU-258) (#171)

Detect a stale pre-OU-222 subagent-stop.js by its structural shape — a
process.exit(2) blocking exit gated behind decision-language patterns —
instead of the printed nudge prose (nudge fired / tier-1), which a reworded
stale copy evaded.

Residual (accepted): a copy that renames DECISION_PATTERN and reworks its
keyword list, or writes the exit code indirectly, would still evade; the
realistic threat is a passively-cached older release (prose drift), not a
hostile rewrite.
