feat: collapse get+search MCP tools into one query tool (OU-323) (#165)

- add queryInputV2 (union of getInput+searchInput) and handleQueryV2,
  dispatching to query.Get or query.Search depending on whether
  query/queries[] is present, else fetch/filter mode; ids[] takes
  precedence over query/queries[] when both are set (id-fetch, not
  search)
- remove getCapability/searchCapability/handlers_get_v2.go/
  handlers_search_v2.go; MCP surface drops from 5 to 4 tools
  (query, kb, backlog, roadmap)
- update serverInstructions, CLAUDE.md tools table, tools_list.json/
  server_instructions.txt snapshots, and the wire/footprint/response-shape
  test suites for the new surface
- tighten TestToolsListFootprintV2's tools/list budget (2200 -> 1800
  tokens; measured 1717 with 4 tools, down from ~1996 with 5)
- port handlers_get_v2_test.go/handlers_search_v2_test.go into
  handlers_query_v2_test.go, covering every domain x mode combination
