shex-serve: --coi flag for cross-origin isolation (design doc phase 4)

COOP/COEP headers on every response (opt-in), making SharedArrayBuffer
available to the served pages -- the suspension primitive the browser
validation debugger (doc/debugger-design.md phases 5-6) will block on.
serve-test covers headers present with the flag (files and errors alike)
and absent without; the design doc gains the Apache .htaccess
equivalent.

docs: mark debugger phases 1-4 shipped in doc/debugger-design.md, fold
tonight's deferred items into doc/refinements-backlog.md (line-granular
gutter breakpoints, web UI for node/predicate breakpoints, worker-app
debugging, console-silence assertions in smoke tests, common REPL
skeleton), and point README at the debuggers.  5076 passing.
