shexmap-simple: step-through materialization debugger (design doc phase 3)

editor-panes: a breakpoint gutter on every pane -- click toggles a red
dot; listBreakpoints()/toggleBreakpoint(pos) expose them -- plus a
shexjs-debug-current highlight color.

ShExMapBaseApp: the 🐞 button next to materialize starts a debug
session over MaterializerDebugger (requires the language-aware editors
for the outputSchema pane).  Gutter breakpoints resolve to the first
constraint on their line via locate.exprAt; ▶⤵⏭⤴⏹ controls step the
session with the current constraint highlighted in the pane and the
thread snapshot (subject, depth, binding frame, consumed, emitted) in
the status line; completion renders the graph through the same path as
materialize (collectMaterializationInputs/renderMaterializedGraph are
now factored out and shared) plus the never-bound/unused-static
warnings; errors anchor as usual.

MaterializerDebugger is exported from the Map module (and so from both
webpack bundles).

packages/extension-map/test/shexmap-editors-smoke-test.js: a jsdom
harness for shexmap-simple.html?editors=1 -- boot with panes over the
ShExMap caches, then a scripted debug session: gutter breakpoint on
:q, step-into pauses at :p, continue stops at the breakpoint with
consumed:1, continue completes with 2 quads rendered; plus stop-on-
demand.  5074 passing.
