fix(review-loop): iteration 1 addressed 4 findings

- [major] [events.go] concatenate inline rich-message spans and newline-separate only blocks, so a word split across formatting nodes rejoins instead of breaking substring/stopword matching and duplicate hashing
- [minor] [events.go] log the rich-message append branch, matching the caption block
- [test-gap] [events_test.go] cover the caption/credit/summary/expression keys and sorted-key ordering
- [test-gap] [events_test.go] cover whitespace-only and padded spans
