docs: correct scheme.md against the conformant resolution surface (#722)

scheme.md still described the pre-#676 (legacy) response shape in three places,
which is how the published method spec inherited the same errors.

- didDocumentMetadata: drop `confirmed` from the standard metadata list. The
  conformant surface strips `confirmed` and `timestamp` and returns them with
  the registration resource (identifiers-router.ts normalizeDidDocumentMetadata
  / buildRegistrationResource).
- didResolutionMetadata: it carries `contentType`, not the legacy `retrieved`
  timestamp. Fixed the revoked-DID example, which showed `retrieved` and
  `confirmed`, and added a worked example of the resolution result.
- Document Accept-header representation negotiation (application/did+ld+json vs
  application/did+json, Vary: Accept) -- implemented but previously unspecified.
- /registration is not the bare didDocumentRegistration object: it is augmented
  with `confirmed` and (for blockchain-anchored registries) `timestamp`.

Verified against tests/gatekeeper/api.test.ts, which asserts each of these.
