fix(plugin): bootstrap detects and reinstalls a stale binary (self-healing upgrade) (#156)

- checkBinary now probes the binary for the claude subcommands hooks.json references (via collectHookCommands) and reinstalls on mismatch, not just when missing
- install() no longer copies a stale local/dev source binary over the release (auto-local falls to release; DEV_BINARY warns)
- actionable stale-repair message
- CLAUDE.md upgrading note (restart, not /reload-plugins)
