refactor(parser): single identity rule for streaming token reuse (#368)

* chore(plans): guard checkpoint 2 for plan 011 — STOP reproduced and diagnosed

Executor halted mid-step-3 at STOP conditions 2 and 3 (redraw-regression
and issue-328 suites red). Guard snapshot-committed the work (35d38d0 on
advisor/011-unify-token-reuse-identity-e10bfcc), reproduced 14 failures,
and diagnosed a suspected plan defect: parser-side reuse returns raw
tokens, severing proxy-space identity that render metadata, keyed each
blocks, and the slugger prefix-skip depend on. Verdict ON TRACK for the
executor; plan amendment pending operator decision.

Operator-approved guard amendment after STOP conditions 2 and 3 fired at
checkpoint 2. The fold-into-update() design severed token identity at
Svelte 5's reactive-proxy boundary; amended contract keeps the identity
decision in the parser (shared predicate + stable prefix length) and the
merge in the component. Done criteria and STOP conditions updated to
match without weakening any quality bar; batch README row moved to
IN PROGRESS; PLAN AMENDED entry logged as guard checkpoint 3.

Operator-approved restatement of the checkpoint-3 step-3 amendment:
title, why-this-matters, scope, git workflow, and step 4 now describe
the proxy-space-merge design directly instead of superseded markers
over the old fold-into-update() text. Done criteria and STOP conditions
unchanged; logged as guard checkpoint 4.
