fix: await Fastify ready when initializing test apps (#3262)

Under Fastify, route lifecycle hooks are only attached once .ready()
resolves, so a supertest request sent after a bare app.init() races app
boot, crashes in Fastify's hook runner and hangs the test until timeout.

Switch the portfolio integration spec to initTestApplication(), document
the rule in AGENTS.md and add a JSDoc warning on the helper.
