fix: sanitize HTTP header values to prevent CRLF injection (CWE-113)

Strip \r and \n characters from header keys, values, and response
reason phrases before writing them to the wire. Without this, user-
controlled input reflected into response headers enables session
fixation, response splitting, and cache poisoning attacks.
