ci: unified release — publish-all.yml, single release.v.* tag

Merge server and client release pipelines into one workflow
triggered by release.v.X.Y.Z. Both server (Cli + Tray) and
client (Android APK) build in parallel, smoke-tested, then
published in a single GitHub Release with all three artifacts.

Delete publish-server.yml and publish-client.yml. Update README
badges (Server + Client both filter release.v.*), rewrite Release
Tag Formats and Internal Version Processing sections. Update
CONTRIBUTING versioning rules — single tag, single version.

Old tags (server.v.*, client.v.*) need manual deletion via
git tag -d + git push origin :refs/tags/.
