test(streaming): extract shared rAF/fake-timer harness across five suites (#367)

The 16ms rAF stub, fake-timer lifecycle, token-cache clear, and the
flushStreamingBatch(50ms) helper were copy-pasted verbatim in five
streaming test files, so the batch-window timing could silently drift
between copies. They now live once in src/lib/test/streaming/harness.ts
(useStreamingTestHarness + flushStreamingBatch); the two suites that
lacked the anti-cascade vi.restoreAllMocks() teardown gain it.

Follow-up to the /simplify reuse observation on #364.
