* refactor(streaming): reset streamTokens by reference replacement (#291 consistency)

- Replace the three streamTokens.length = 0 reset sites with
  streamTokens = [], unifying all writes to the #291-mandated
  reference-replacement pattern
- Add characterization tests pinning reset-to-empty DOM behavior for
  imperative resetStream('') and prop-driven source: '' while streaming
- Mark plan 002 DONE in the redraw-hardening batch README

/simplify altitude finding: the rule lived only inside
applyStreamingSource, ~280 lines from the farthest reset site it
governs; the declaration is the one place every writer sits under.
