* docs: make dependabot unblocker failure list pluggable

Split the unblock-dependabot-pr skill into a thin engine (SKILL.md) plus an
appendable failure-pattern catalog (references/failure-patterns.md), so adding
a newly discovered failure mode is a single-row edit instead of a rewrite of
the skill body. Promote the needs-rebase pattern to a Phase-0 guard and add a
retry-budget guard that escalates a PR to human review after three automated
unblock attempts.
