test: raise C# SDK coverage to ~94% (#9)

Add focused xUnit tests exercising real behavior across the previously
under-covered surface: the HTTP client factory (cookie jar / no-TLS-bypass /
custom-CA callback), gRPC channel construction, AxiamClient auth state machines
(refresh/logout/MFA/login field mapping), the REST message handler
(tenant/auth/CSRF injection + 401->refresh->retry), ErrorMapper status tables,
ReplayGuard extraction branches, gRPC authz client construction/identity
resolution, AMQP consumer disposal, core value types, and the ASP.NET Core DI
extensions. All tests use mocked transports; no live services.

Merged line coverage 73.3% -> 93.7%. No production source changed.
