test: add error-handling and dedup tests, simplify paginate (#132)

- Add unit tests asserting DynamoDB errors propagate from getItem,
  putItem, deleteItem and updateItem.
- Add unit tests verifying batchGetItems deduplicates repeated keys into
  a single request and splits >100 unique keys across multiple batches.
- Rewrite the internal `paginate` helper from a reduce-with-index-math
  accumulator to a straightforward slice-based loop (same behaviour,
  clearer and avoids per-element Math.floor).
