feat(cli): bind --read-only to an env var on ServerCmd (MC-52) (#33)

- add Server.ReadOnlyEnv: opts a built command into read-only mode when the named env var is set to a truthy value (1/true/yes/on, case-insensitive, trimmed), OR'd with --read-only
- add unexported envTruthy helper for the truthy-token check
- default (empty ReadOnlyEnv) keeps MC-46 behavior byte-identical
- regen cli/README.md via docsgen
