feat(mcpkit): startup tool gate — App.Gate/ReadOnlyMode + App.BlockGroups (MC-44) (#27)

- add App.Gate(opts...GateOption) and ReadOnlyMode() to hard-block every non-ReadOnly pending tool at finalize time
- add App.BlockGroups(groups...string) to hard-block pending tools tagged with a named group
- both are pre-start-only: erroring instead of silently no-op'ing once registry.started is true
- wire the gate predicate into registry.finalize at the MC-43 seam: a gated-off tool is never registered against the live server and never recorded in byGroup
- cover both axes individually, combined, the no-gate regression case, and the post-Connect error path
