chore(plugin): remove ported Node hook scripts (#149)

- all 7 hooks now run as native Go handlers in the binary (`ouroboros claude hooks <event>`); hooks.json already points every non-SessionStart event there
- delete the now-inert node hook scripts + their tests/fixtures; only bootstrap.js remains in plugin/scripts/
- plugin test suite trimmed to bootstrap coverage (45 tests)
- fix bootstrap.test.js's stale-subagent-hook tests: they read the repo's own scripts/subagent-stop.js as a clean reference, which is now deleted; use inline clean content instead (the OU-254 stale-cached-install guard itself stays, still useful for pre-existing user installs)
- update CLAUDE.md, persist SKILL.md, and tests/README.md references to the deleted scripts
