Release modernization, and publish to PyPI from CI via trusted publishing (#175)

Replaces the manual build+twine release process with a GitHub Actions workflow (`publish.yml`): publishing a GitHub release with a `vX.Y.Z` tag builds the package and uploads it to PyPI via trusted publishing. Version still comes from the git tag; the releases page serves as the changelog.

Also removes files the old process left behind (`Makefile`, `docs/Makefile`, `setup.py` stub), documents releasing and docs-building in the README, and updates the tox envlist.
