feat: restore server instructions and tool annotations on the mcpkit server (OU-5a) (#138)

- bump mcpkit pin to v0.0.0-20260711191839-834dc899908b (MC-25: Info.Instructions,
  mcpx.ToolAnnotations, mcpx.BoolPtr)
- move serverInstructions from server.go into serverv2.go (V2's new home; buildServer
  still references the same const, same package) and pass it via
  mcpkit.Info.Instructions on buildServerV2
- restore per-tool annotations on the 5 dark tools, matching buildServer's
  toolAnnotation(...) mapping exactly: get/search ReadOnlyHint, kb IdempotentHint,
  backlog/roadmap DestructiveHint
- add wire tests: an in-process client (mcpx.InMemoryPair, mirroring mcpkit's own
  mcpx_test.go pattern -- testkit.Harness doesn't expose ClientSession) asserts
  InitializeResult().Instructions == serverInstructions; a tools/list assertion via
  testkit checks each tool's Annotations against the mapping above
- closes OU-293 (annotation support landed in mcpkit + hints restored)
