feat: add panic-recover hook + mcpx result builders (MC-8) (#8)

- mcpx: TextResult/ErrorResult build CallToolResult without importing
  go-sdk, pairing with the existing ResultText.
- mcpkit.AddTool: wrap every handler in a panic-recover at the
  tool-registration chokepoint, converting a panicking tool into an
  IsError result naming the tool and panic value instead of crashing
  the process; happy-path handlers are unaffected.
- mcpx: probe test documents go-sdk's error-as-tool-result semantics —
  a plain Go error returned from a typed handler already surfaces as
  an IsError tool result, not a protocol-level error.
