Fix Doxygen gh-pages deploy: add missing header.html, drop beman logo refs

The deploy job has failed on every run since the repo was seeded from the
example project. The Doxyfile referenced files that were never copied over:

  error: tag HTML_HEADER: header file 'docs/header.html' does not exist

- Add docs/header.html (from the example project; wires up the awesome
  darkmode toggle fixpoint already ships).
- Clear PROJECT_LOGO / PROJECT_ICON, both pointing at a nonexistent
  docs/beman-logo.png (stale beman branding).
- Add a .gitignore exception so docs/header.html isn't caught by *.html.

Verified locally with doxygen 1.9.8: exit 0, index.html generated, no
missing-file errors.
