fix(plugin): guard against stale subagent-stop nudge that destroys reports (#114)

- add OU-254 regression test locking in OU-222 no-subagent-nudge behavior for decision-language final messages
- add bootstrap.js SessionStart check (isStaleSubagentHook/checkSubagentHookStale) that warns when a cached subagent-stop.js still nudges subagents, fail-open, always exits 0
- add stale-hook fixture + unit/CLI tests covering both stale and clean subagent-stop.js
