Gate per-process and cgroup metrics sampling on start-time identity

Close the PID-reuse window where an exited process's pid gets recycled by an unrelated process before a metrics read lands, by capturing a process-start-time identity token at spawn and reconfirming it (via the same pinned handle on Windows, /proc/<pid>/stat field 22 on Linux) before trusting any later sample; cgroup group-stats folding reconfirms both identity and current cgroup.procs membership per member, skipping unconfirmed members instead of silently under- or over-counting.
