feat: add document highlight support (textDocument/documentHighlight)

Highlights every same-file occurrence of the $variable, function, or
namespace prefix under the cursor, tagging declarations as Write and
usages as Read. Reuses the classification and occurrence-walking logic
already built for find-references and rename, restricted to the current
file per the LSP spec.
