refactor: replace magic visibility strings with (string, bool) and use rune-based widths

R-2: LayerHandler and every Print*Layer/LookupCountry now return
(string, bool) instead of relying on sentinel strings ("", "invisible")
to signal 'nothing to render'. Removes magic-string comparisons from
processAndPrintPacket and makes the 'no content' case a checked, typed
contract instead of a string convention.

R-4: RenderBlock and PrintHorizontalBlocks size their boxes using
utf8.RuneCountInString instead of byte length, so multi-byte UTF-8
content no longer inflates the computed box width.
