Support multi-row Azure Table v2 blocks (#158)

* Support multi-row Azure Table v2 blocks

Implement deterministic multi-row row-set storage for lexongraph-block-store-azure-table-v2 so logical blocks larger than one Azure Table row remain supported without changing the public API.

This updates the v2 spec package, adds root-plus-continuation row encoding with per-row chunking, treats the root row as the publication commit point, extends enumeration to validate referenced continuation rows, and expands the live Azure test to cover the new multi-row path.
