* chore(test): gate coverage; declare engines; cover rAF fallback

- vite.config.ts: add coverage thresholds at measured floor plus a
  text-summary reporter so coverage cannot rot silently
- package.json: declare "engines": { "node": ">=22" } to match CI
- SvelteMarkdown.test.ts: cover the rAF-absent streaming fallback and
  its timeout-cancel branch

Implements plan 010 (streaming-component-hardening).

ON TRACK against snapshot 159ff06. Records the reproduced done
criteria, lcov evidence that the timeout-cancel branch is genuinely
covered, and two findings for the operator: a stale plan excerpt
(renamed since the Planned-at SHA) and tight coverage-threshold
headroom on functions/lines.

Operator-approved amendment following guard checkpoint 1.

- Correct the Current state excerpt: scheduleAppendFlush /
  flushPendingAppendChunks were renamed to scheduleStreamFlush /
  flushPendingStreamChanges by plans 007-009, so the drift STOP fired
  on a cosmetic diff. STOP wording now distinguishes a pure rename
  from a behavioural change.
- Require coverage floors >=1 point below measured rather than a bare
  integer round-down; functions: 96 vs measured 96.19% left only one
  uncovered function of headroom.
- Add a live-gate check so the threshold criterion cannot be met with
  inert config.
- Re-stamp Planned at to fbc1ea5; re-open 010 for the threshold edit.
