feat: composite optional layout template segments (#184)

* feat: add composite optional layout template segments

Extend the custom template engine with composite tokens such as
{Vol series_number:02 - } and {[narrator]}, numeric :02 padding,
and the book_title alias. Empty slash-separated layout segments
continue to be omitted when a field renders blank.

Adds TUI custom layout editing, docs/changelog updates, and tests
written before implementation.

Resolves #183

* refactor: remove book_title template alias

Use {title} only in templates, docs, defaults, and tests.

* fix: use per-file track prefixes when organizing multi-chapter books

Multi-chapter directories were applying the first file's track number to
every moved audio file. Resolve embedded per-file track metadata instead,
skip redundant prefixes for single-track audiobooks, and leave non-audio
files unchanged.

* fix: render only first narrator for {narrator} template field

{narrator} now resolves to the first narrator while {narrators} keeps the
full comma-separated list. Document the distinction in layout template
references and add regression coverage.

* fix: normalize dash field names and honor composite fallbacks

Normalize dashes to underscores when scanning composite template content
so field references like series-number match known fields. Apply pipe
fallback values when any inner composite field is empty, matching simple
token behavior.
