Fix review findings: examples, guava, simulator

- write-behind-rxjava: run the coalescer inside the subscribe
  try/catch and add an onError backstop so a throwing coalescer
  drops its batch instead of silently tearing down the stream
- coalescing-bulkloader: validate maxSize/maxTime/parallelism in
  build(); cover the throwing-mappingFunction and missing-field paths
- guava: extract the unsupported-loadAll per-key fallback into a
  shared CaffeinatedLoader.loadSequentially helper
- indexable: extract the index-teardown into removeIndexes()
- simulator: pass Locale.US in the hill-climber debug/trace printf
