feat(deploy/noderunner): add add-observability stage (prometheus + grafana)

Two containers, no funding, no external creds. Per operator preference,
admin UIs go on Tailscale only — the stage's checkpoint is confirming
Tailscale-only exposure (via `tailscale serve` or bound caddy on the tailnet
IP), never publishing Grafana or Prometheus on the public Caddyfile.

- stages/add-observability.md: procedure with Tailscale checkpoint,
  Prometheus target verification, Grafana bootstrap notes
- SKILL.md + README.md: register the new subcommand + file listing
