fix(extensions): handle dangling $ref in DeletedStateExtension

* Catch AttributeError when a source entity's $ref resolves to None (e.g. purged by VIAF cluster-merge cleanup), instead of crashing on `.get()`
* Fall through to the next entity instead of aborting propagation when one ref is dangling
* Add unit tests: a dangling ref alongside a valid deleted ref, and the case where every linked ref is dangling
