Refactor pi_check using fractions.Fraction (#16361)

* Refactor pi_check using fractions.Fraction

Replace FRAC_MESH/RECIP_MESH and hand-rolled fraction checks with
Fraction.limit_denominator matching, preserving MAX_FRAC and MAX_PI_FRAC
behavior. Fixes #16356.

* Address review: unify MAX_FRAC=99 and use Fraction for whole multiples of pi

* Fix whole-multiple pi matching and update QASM boundary tests for MAX_FRAC=99

* Fix ruff RUF046 in pi_check whole-multiple coef

* Simplify pi_check to three Fraction-based checks per review

* Add release note for higher pi fraction integers in visualization
