fix: use app home dir for git commands to work regardless of cwd

The app starts with cwd '/'. Running git commands without an explicit
directory fails silently and fall back to 'unknown'.

Use 'git -C <app_home>' to point git at the application directory
regardless of the process working directory. Combine hash and date into
a single git call to reduce subprocess overhead.
