refactor(prebuilt): decompose buildPiAgentGraph into cohesive helpers

Extract the 270-line buildPiAgentGraph into single-responsibility helpers
(mergePiAgentState, newPiAgentNode, newPiToolsNode, routePiAgent), leaving
the builder as pure graph wiring.

Also fix a go vet self-assignment in Reset() that silently dropped tools:
capture-then-restore now honors the "keep the tools" intent.
