Pre-release polish: drop personal fixture, finish rename, scope hint (#26)

Final pass before the public v0.1.0 cut, from a ruthless review plus
the on-target user's first-run report.

- Remove the live Google Doc fixture that hardcoded a personal doc
  ID (the "Loose Brick" story) and its title assertion. Now that the
  repo is public, tying personal fiction to it reads as stray debug
  code; the ignored test added little (it never ran in CI). The
  URL-parsing unit tests keep the import feature covered, using a
  synthetic doc ID.
- Reword the last stale-name comment: "A configured writings root"
  becomes "A watched root directory."
- On a failed push, if the error is a 403, suggest the fix. A token
  can be valid yet lack the `gist` scope (common when `gh auth
  login` predates caring about gists), which otherwise faceplants
  as a bare 403 on the very first push. Point straight at
  `gh auth refresh -s gist`.

Two critic findings were checked and left alone: edition 2024 is
correct (rust-version 1.85 declares it; 2024 stabilized there), and
CONTRIBUTING/issue templates are skipped to match the edikt
standard, which has none.
