Correct stale docs surfaced by the lib review

- :tolerance and :max_iterations option docs: tolerance thresholds the rate
  step/bracket width (not the NPV), and the solver returns the bracketed
  estimate on exhaustion rather than "giving up".
- Newton moduledoc: a step leaving the bracket becomes a bisection step; a
  genuine overflow maps to :did_not_converge, not a bisection fallback.
- ytm: drop the comment claiming the annualization isn't a second rounding.
- Refresh CLAUDE.md for the multi-module layout, both solvers, optional
  Decimal/ex_money, and the 100%-coverage gate.
- README: bump the finance_rustler snippet to ~> 1.6 (the Hex version badge
  is dynamic and already current).
